All these issues were verified against LRM version D4.3a which
is now ready for IEEE ballot resolution. Two minor typos were
discovered, which can be fixed in later editing. Also, some
examples don't follow the recommended conventions for the case
of identifiers. The examples came from different sources and it
is impractical to rewrite them.

ACTION: On to ballot resolution!
