Change to LCS-2006-006: IR2013

Folks,

While working on another LCS, I noticed an oversight in LCS-2006-006:IR2013.
In that LCS, we added specification that a subtype check be done in a signal
assignment to check that the value of each waveform element belongs to the
subtype of the target. However, we forgot to delete Note 5 that says such a
subtype check is not done. The revised LCS is
