Revised IR2119 for review

Folks,

Attached is a revision of IR2119, "Can't declare a protected type and object
of that type in a single package," with additional notes addressing Jim's
and John's negative-ballot comments. If this is ok, we can fwd to the
VHDL-TC for requirements analysis.
