1. Open Bugzilla issues for version D4.0

The Accellera standard has been approved and passed on to the VASG.
The few open issues are a bottleneck to language approval, so we need to
resolve them as soon as possible.  All of us have accounts on the website,
so try to look at the open issues before the meeting.

Peter gave me some good feedback on 2126--I will try to get an updated

The ISAC has voted to approve 2128, but Jim had some reservations, so we 
will briefly discuss this at the meeting (time permitting)

Suspended until Accellera/IEEE process completed on current version of LRM.
