[sv-cc] Changes to SV-CC database items

Hi All,

I have started to get the SV-CC database items into a reasonable working
state. To accomplish this, I moved all items that were assigned arbitrarily
to Joao to unassigned, and made changes to reflect what we had decided in
our meetings. Please find below a complete list of the changes that I made:

   - Item 124: assigned to vellenga. Added PDF version of proposal. Marked as Resolved
                with Resolution = fixed.
   - Item 125: Re-opened because it was not resolved properly. Assigned to vellenga.
                Added PDF version of proposal. Marked as Resolved with Resolution = fixed.
   - Item 158: assigned to michael
   - Item 162: assigned to vellenga. Added PDF version of proposal. Marked as Resolved
                with Resolution = fixed.
   - Item 202: assigned to Ralph
   - Item 205: assigned to Ralph; moved from Enhancement to Errata

In the next few days, I will be continuing to bring the database up to date.
