Header file tlm.h

All,

The name of the header file "tlm.h" is now set in stone, and all the names
in that header are in namespace tlm.

Do we want to add a header file "tlm" for consistency with #include
"systemc"? This new header "tlm" would simply #include "tlm.h".

Does that work?
