Graham has also been looking at an initial proposal on cleaning up the BNF to make it consistent and incomplete within itself and also between Appendix and BNF references inside sections. He will sending the mail out shortly (if he hasnt already done so).
 
If people are still interested in discussing the list of issues sent out in the table and also discuss the proposal to cleanup the inconsistencies in the BNF let me know.
