General Description:
When a PAD is added or removed from a Concept schematic with
HDL direct enabled, or if the signal names change between the
IPAD and IBUF or OPAD and OBUF, saving the design with HDL
Direct enabled may give the following error:

Error #171: Port exists in entity declaration but not
on the schematic. The entity declaration may need to be
updated. Port name: .......
Would you like to like to overwrite the entity declaration
<path to vhdl.vhd> with a new entity declaration derived
fromthis file schematic?[y/n]
</output>
Are you sure? [y/n]
Solution 1:
Simply type "y" in response to both questions.
This should really be a warning, and will always
occur when adding/deleting/changing external ports.
The problem has been reported to Cadence.
