I searched for "of of" and found another one in glossary item I.343.
This is a little embarrassing,  because it was reported in Bugzilla 116.
Instead of fixing it, we merely annotated it.

Annotated version p16 line 45 has Boolean, which should
be BOOLEAN. I also noticed in the same example that "Bit" occurs where
we prefer "BIT". In searching for BIT I discovered many instances with wrong
capitalization. If we defer this, we should probably do an exhaustive 
check of
capitalized identifiers later on.

Annotated p122 line 61 has another instance of  "is is" which should
be "it is".

With this in mind I am willing to consider D4.3a as  "good enough"
to recirculate and to publish.  Also, we might be allowed to fix the
two true typos "is is" and "of of" as part of some sort of editorial 
process.
