$table_model LRM 2.3 update

Hi,

The new text for $table_model in 2.3 is available at this temporary location

Geoffrey will upload the file to the usual location but probably not before
the meeting tomorrow. Some diagrams need to be filled out and a couple more
examples are needed but otherwise it's complete.

The quick overview - $table_model now supports:

- linear, quadratic, cubic splines, and a local cubic interpolator for all
dimensions
- exact and closest lookup modes
- constant extrapolation
- multiple dependents
- ability to ignore input columns
- doc on isolines in order that all of the above make sense

A typical control string might look like this,

$table_model(filename, V(1),..., "I, 1LC, 3LL;1");

That means "Ignore column 1, use linear and constant extrapolation on the
outer dimension of the data, use cubic splines on the inner dimension with
linear extrapolation on both ends. Do this interpolation for the third
dependent variable in the file."
