The AMS LRM is somewhat vague about what mode is used: you get an error if the file can't be opened for writing, so I guess that means the mode in AMS is "w" (not "a") -- but this should be noted explicitly. I had noticed the omission of $fwrite, etc. while I was doing LRM 2.2, but thought that these would get added to LRM 2.3 when aligning with 1364-2001.
