Hi all,

please find attached the 'Inclusion Appendix', as received from Joe
and edited to remove the definition of source code integration (as we have
decided earlier in one of the polls).
The only changes I did was to completely remove all definitions related
to source code integration, I have used the TechWriter comments provided
by Joe, so you see what has been removed.
I did not:
 - remove the SV_LIBRARIES header from the Bootstrap file (I thought I have
   seen this once, don't remind from whom); what is the opinion of the team about it.
   Keep it or kill it?
 - remove the switch names; this would make this stuff unreadable. It is clearly
   said, that the switch name is just a recommendation. If vendors want to
   distinguish themselves on switch names, fine with me ;-)
 - remove the notion of switches completely, as Francoise requested. This would
   make the whole appendix void and useless. I am sorry.
