This revision includes the corrections sent by Andrzej, John & Doug.
(I skipped over some of the cosmetic ones, specifically the reformatting
of the comments, in the interest of saving some time)
In addition, it also includes the following updates:
- "virtual function" update: multiple exports in different scopes can share same cname as long as type signature is identical
- has the new put/get user data functions
- same rewording of the type layout rules to make them clearer
