[sv-cc] Please mention your name while filing a bug

As you might have discovered by now, since we are using company
based accounts, there is no way to find out who filed a bug in
our database unless you explicitly write your name in the bug
report.

So, my request is: please put your name in the bug report. This
will help us to contact the right person if we need to.
