[sv-cc] 1343 unpacked array index correspondence

A new bugnote and proposal were added to the Mantis item 1343.
 
The new material tries to
. clarify that existing text is about mapping SV import decl formals to
user C formals
. add new text to describe mapping SV actual arguments to SV import decl
formals.
