[sv-cc] Packed array proposal (Mantis #1230) updated

All-
I have uploaded a new version of the packed array proposal to address
the friendly amendments as noted (attached here also for convenience).
Also, I have made a few other minor corrections (one mistake, and one
for consistency).  I have outlined all changes below.  Please review
these, and let me know if this is acceptable as-is, or if we need more
corrections and/or need to table this for our next meeting.

Changes made:

1) 36.15 Nets, diagram p. 1:    vpiElement transition changed to double
arrow
                                (corrects mistake- was single arrow)

2) 36.15 Nets, Detail 1, p. 2:  Added examples as requested

3) 36.15 Nets, Detail 22, p. 3:  Added reference to example in detail 29
   (so as to be consistent with similar reference requested in new
packed array var detail 6).

4) 36.21 Typespec, Detail 8, p. 14:  Corrected reference to "arr" as a
4x2 array (now 4x3) in
                                     2nd paragraph.

5) 36.22 Packed array variables, Detail 6, p. 16:
                                 Added reference to example in detail 3,
p. 15 for clarity,
                                 as requested.
