[sv-cc] Proposal (combined) for issues 121, 122, 123

I couldn't really separate the formal proposals for
these three issues. I have therefore posted a single
.fm file three times, once for each issue. For
those of you who can't read Framemaker, I have also
posted a .pdf file on 123 (although it lacks the
colors) and an HTML file with its subordinate files,
also on 123. The HTML file has the colors but has
other problems.
