[sv-cc] Updated 265 (Assertion VPI)

Hi All,

I've updated 265 with the following SV_LRM_265.pdf (1,456,538 bytes)

Sorry for the redundant file attachments. Since I am using a guest account I
am unable to delete the old ones (someone please help with that). 265 has
screen grabs that have been annotated with the correction. At least this is
good for the time being to get the idea across, a soft version can be used
later (I don't have one) to make the corrections and post a new proposal.

265 also incorporates "0000266 SV-CC minor assigned (bassam) 10-22-04
[SV-CC] sv_vpi_user.h has an unused vpiSequenceSpec" in one of it's changes,
seemed better to do it this way and avoid a race in the two.

** Jim/Francoise/Joao/Michael/Doug/all please take a look at this and offer
feedback.

** Someone please help delete the older version of proposals for: 265, 67,
69, 264.
