[sv-cc] Proposals for #63, #158

Hi,
- I believe the updates of Jim for #301 have rendered most of the errata
#63 superfluous, (Francoise could you please verify this).
  A proposal has been uploaded to repair the contradicting note #11.
- A proposal is now available for #158.
- I have further added a new errata #330 for applying #288 to #205 as we
have discussed in our latest confenence call.
   A proposal has been added to this errata. Please review for tomorrows
conference call.
